Responsibilities

• Conduct initial patient assessments and develop individualized care plans tailored to patients' health needs.
• Provide skilled nursing care, including wound care, injections, medication management, and monitoring vital signs.
• Educate patients and their families on health management, medications, and preventive care.
• Collaborate with the healthcare team to coordinate care and communicate patient updates and changes.
• Document all patient care activities, assessments, and communications in a clear and timely manner.
• Perform patient evaluations on a regular basis to ensure their ongoing health and recovery.
• Assist patients with activities of daily living as necessary to support their independence.

Requirements

• Registered Nurse (RN) or licensensed Practical Nurse (LPN) licensed in the state of practice.
• Bachelor's degree in Nursing (BSN) preferred; associate degree acceptable for RNs.
• Previous experience in home health care is highly desirable.
• Strong assessment and clinical skills to evaluate patient needs effectively.
• Excellent communication and interpersonal skills to build rapport with patients and families.
• Knowledge of medical equipment, disease management, and home care regulations.
• Ability to work independently and manage multiple cases effectively.
• Current CPR certification and other relevant certifications preferred.
