Job Title: Chaplain/Bereavement Coordinator
Location: Sandy Springs, GA 30358

Our Georgia hospice client, in partnership with our Healthcare Consulting & Staffing firm specializing in Hospice, Home Health, and Homecare, is seeking a dedicated and compassionate Chaplain/Bereavement Coordinator to join their healthcare team. In this role, you will provide emotional and spiritual support to patients, families, and staff while coordinating bereavement care services for families and loved ones experiencing loss. You will work closely with interdisciplinary teams to ensure the holistic needs of patients and their families are met.
ResponsibilitiesSpiritual Care: Provide spiritual counseling and support to patients and families, respecting their individual beliefs and traditions.
Bereavement Coordination: Develop and implement bereavement care plans tailored to families' and loved ones' needs, offering ongoing support after a patient's death.
Assessment: Assess patients' and families' emotional, spiritual, and cultural needs, integrating findings into the overall care plan.
Team Collaboration: Collaborate with the interdisciplinary team, including social workers, nurses, and medical directors, to ensure holistic care delivery.
Crisis Intervention: Offer support during times of crisis, helping patients and families navigate grief, loss, and end-of-life transitions.
Education: Provide grief and bereavement education to patients, families, and staff as needed.
Community Outreach: Represent the hospice organization in the community to provide information and build relationships with local spiritual and cultural groups.
Documentation: Maintain accurate and up-to-date documentation of all patient interactions, assessments, and bereavement activities in electronic health records.
Compliance: Ensure all activities adhere to hospice regulations and best practices for bereavement and spiritual care.
Education: Master's degree in Divinity, Theology, Pastoral Counseling, or a related field preferred.
Certification: Certification as a Chaplain or Bereavement Counselor preferred (e.g., APC, NACC, or similar).
Experience: Hospice experience required; prior experience in grief counseling or chaplaincy strongly preferred.

Compassionate and empathetic approach to care.
Strong interpersonal and communication skills to interact effectively with diverse populations.
Ability to work collaboratively within a multidisciplinary team.
Excellent organizational and documentation skills.
Technology: Proficiency with electronic health records (experience with HCHB EHR preferred).
Transportation: Reliable transportation and auto insurance required.
State Specific: Documentation of ordination or commission by a recognized faith group; completed at least one unit of clinical pastoral education from a nationally recognized provider.
